Dental: Choose the Core Dental Plan or the Buy-Up Dental Plan. Both plans offer the same benefits with two differences: the Buy-Up plan will pay up to $1500 each year in dental expenses where the annual max on the Core plan is $1000, and if you elect the Buy-Up plan, your dependent children up to the age of 19 can receive Orthodontia benefits of $1500. |

Vision: With a small co-pay, you can receive an exam, lenses, frames or contacts every 12 months. Please reference the coverage summary for details. We contract with independent Optometrists and Ophthalmologists, but you can visit any eye care professionals you wish. |

Supplemental Term Life Insurance: Supplemental Life and/or Accidental Death & Dismemberment Insurance can be purchased during the annual Open Enrollment period for yourself and for your spouse or dependent children. Evidence of Insurability is required. |
